We invoice many insurance companies directly. At the time of calendaring a deposition with our office, please provide our scheduling department with the following:

• Claim/Policy Number
• File Number
• DOL
• SALN
• CR
• Insured
• Any other pertinent information required by the insurance company to enable them to process our invoices for
  payment.
